Epidermolysis Bullosa: Basic and Clinical Aspects

Because skin blisters are the initial manifestation of epidermolysis bullosa (EB), patients invariably present to the dermatologist for diagnosis and treatment. However, EB is a systemic disease whose management requires input from clinicians in virtually all fields of medicine, including pediatri­ cians, surgeons, dentists, gastroenterologists, hematologists, otorhinolaryn­ gologists, dietitians, and physical therapists, to name a few. Because EB is a rare disease, few clinicians are familiar with it, and many recoil at the pros­ pect of caring for individuals covered with blisters caused by a disease they know little about. For patients, insult is thus added to injury and they feel abandoned, neglected, and frustrated. One way to remedy this deplorable situation is to provide clinicians with a compact source of information de­ tailing the principles of EB diagnosis and treatment. This text seeks to fulfill this role. From 1986-1991, The Rockefeller University Hospital has been the co­ ordinating center of the National EB Registry. Supported by The National Institutes of Health, this Registry consists of four university centers* commit­ ted to collecting clinical data concerning diagnosis, treatment, and epidemio­ logy on all American EB patients. As of April 1992, nearly 1,799 EB patients have enrolled nationwide. The Registry is now in its second five-year phase of operation.

It is almost self-evident that surface and interface science, coupled with the electronic structure of bulk materials, playa fundamental role in the understanding of materials properties. If one is to have any hope of understanding such properties as catalysis, microelectronic devices and contacts, wear, lubrication, resistance to corrosion, ductility, creep, intragranular fracture, toughness and strength of steels, adhesion of protective oxide scales, and the mechanical properties of ceramics, one must address a rather complex problem involving a number of fundamental parameters: the atomic and electronic structure, the energy and chemistry of surface and interface regions, diffusion along and across interfaces, and the response of an interface to stress. The intense need to gain an understanding of the properties of surfaces and interfaces is amply attested to by the large number of conferences and workshops held on surface and interface science. Because of this need, the fields of surface and interface science have been established in their own right, although their development presently lags behind that of general materials science associated with bulk, translationally invariant systems. There are good reasons to expect this situation to change rather dramatically in the next few years. Existing techniques for investigating surfaces and interfaces have reached maturity and are increasingly being applied to systems of practical relevance. New techniques are still being created, which drastically widen the scope of applicability of surface and interface studies. On the experimental side, new microscopies are bearing fruit.

This monograph deals primarily with the kidneys, ureters, and urinary blad­ der. The kidneys are retroperitoneal structures that parallel the psoas muscle. The left kidney is usually slightly higher than the right and is slightly more medially located. The vertical axis of the kidneys, when compared with the midline, is about 20°. There is often considerable mobility of the kidneys as a result of respiration and body position. Several centimeters of excursion have been demonstrated on deep inspiration or in the upright position. During late embryological development, each kidney occupies the flank region, capped by the liver on the right side and the spleen on the left. Abnor­ malities of the liver and spleen can affect the position of the kidneys. Also, retroperitoneal masses may displace the kidney. A palpable abdominal mass which radiographically may appear to be an intraperitoneal structure can be accurately localized as a retroperitoneal tumor by observing displacement of the kidney, particularly if the kidney is pushed caudally and medially, superi­ orly and laterally, or medially. Anomalies of the kidneys include abnormal position, abnormal number, changes in shape, and alterations in the inner structure that may affect the renal parenchyma, the pelvocalyceal systems, or both.
