Impacts of Medications on Male Fertility (Advances in Experimental Medicine and Biology #1034)

by Erma Z. Drobnis Ajay K. Nangia

The over-arching goal of this volume is to help infertility practitioners evaluate and manage their patients with poor semen quality. The authors review the existing literature on the effects of medications on male fertility, and provide detailed information about what is known, giving the number of individuals and population characteristics for studies of medication effects on male fertility. Medications are designed to treat illness and reduce symptoms, but all have undesirable adverse effects such as headache or stomach upset. Some adverse reactions can even be life-threatening, so it is no surprise that some drugs have negative effects on male reproduction. Medical practitioners rarely consider a man’s reproductive plans when prescribing medications. Men are routinely treated with drugs that can impair or abolish fertility.Although practitioners in the field of reproductive medicine generally realize that certain drugs impact negatively on reproductive health, there are limited resources providing evidence-based knowledge useful in counseling patients. Tables throughout this volume summarize the information for each drug, providing a handy reference for clinical use.

Intracytoplasmic Sperm Injection: Indications, Techniques and Applications

Topics covered include the role of reproductive genetics, the effects of paternal age, the assessment, development and selection of functional human spermatozoa (e.g., Comet assay), both naturally and in the laboratory, and the procedures by which these sperm are injected directly into an oocyte. Comparisons are made between efficacy rates of ICSI versus conventional IVF strategies, and clinical approaches to fertilization failures following ICSI are presented, and intracytoplasmic morphologically selected sperm injection (IMSI) is discussed in detail in a chapter of its own.Bringing together the latest evidence and written by international leaders in the field, Intracytoplasmic Sperm Injection: Indications, Techniques and Applications is a unique and accessible resource for reproductive endocrinologists, andrologists, geneticists and infertility practitioners and researchers alike.

Maternal Death and Pregnancy-Related Morbidity Among Indigenous Women of Mexico and Central America: An Anthropological, Epidemiological, and Biomedical Approach (Global Maternal and Child Health)

by David A. Schwartz

This ambitious sourcebook surveys both the traditional basis for and the present state of indigenous women’s reproductive health in Mexico and Central America. Noted practitioners, specialists, and researchers take an interdisciplinary approach to analyze the multiple barriers for access and care to indigenous women that had been complicated by longstanding gender inequities, poverty, stigmatization, lack of education, war, obstetrical violence, and differences in language and customs, all of which contribute to unnecessary maternal morbidity and mortality. Emphasis is placed on indigenous cultures and folkways—from traditional midwives and birth attendants to indigenous botanical medication and traditional healing and spiritual practices—and how they may effectively coexist with modern biomedical care. Throughout these chapters, the main theme is clear: the rights of indigenous women to culturally respective reproductive health care and a successful pregnancy leading to the birth of healthy children.A sampling of the topics: Motherhood and modernization in a Yucatec village Maternal morbidity and mortality in Honduran Miskito communities Solitary birth and maternal mortality among the Rarámuri of Northern Mexico Maternal morbidity and mortality in the rural Trifino region of Guatemala The traditional Ngäbe-Buglé midwives of Panama Characterizations of maternal death among Mayan women in Yucatan, Mexico Unintended pregnancy, unsafe abortion, and unmet need in GuatemalaMaternal Death and Pregnancy-Related Morbidity Among Indigenous Women of Mexico and Central America is designed for anthropologists and other social scientists, physicians, nurses and midwives, public health specialists, epidemiologists, global health workers, international aid organizations and NGOs, governmental agencies, administrators, policy-makers, and others involved in the planning and implementation of maternal and reproductive health care of indigenous women in Mexico and Central America, and possibly other geographical areas.

The Complete Guide to Kettlebell Training

by Allan Collins

A kettlebell is a weight that looks like a cannonball with a handle. Originating from Russia, kettlebell training is now a global phenomenon with clubs, gyms and individuals investing in equipment and training. Weights range from 4kg to 48kg. They differ from dumbbells and barbells in that the centre of mass is offset from the handle, so the weight constantly pulls against your hand, improving coordination and anaerobic fitness and strengthening your core muscles. Kettlebells can be used for a variety of purposes - from weight loss to strength training - by people with a range of fitness levels. Popular with celebrities such as Penelope Cruz and Sylvester Stallone, they are also used for training players at Chelsea and Liverpool football clubs, and in 2010 the British Army got on board when their PTI corp (responsible for the Army's physical training) signed up to a 30-day course designed and implemented by the author. The book includes information on the benefits of kettlebell training and how to use the equipment safely, and provides warm-ups, drills and training programmes for beginner, intermediate and advanced levels. From buying a kettlebell to training for weight loss, it's a comprehensive guide to a popular new fitness tool.

Dysmenorrhea and Menorrhagia: A Clinician’s Guide

by Roger P. Smith

Dysmenorrhea is estimated to effect upwards of 70% of women during their lifetime and “heavy menstrual periods” is one of the most frequently given reasons for hysterectomy in the United States. Despite the dramatic growth of dysmenorrhea treatment with non-steroidal anti-inflammatory agents (NSADs) in the 1980s, young women continue to go undiagnosed and under treated. The diagnosis and management of heavy menstrual bleeding has even poorer general understanding because of its more complicated etiologies. With a working knowledge of the underlying pathophysiology, all clinicians should be able to establish both the diagnosis and a credible treatment plan that will result in a significant chance of clinical improvement for most patients. This book addresses the pathophysiologic underpinnings of both dysmenorrhea and menorrhagia and provides clinicians with practical approaches to establishing the diagnosis, proven tactics for successful therapeutic interventions and dealing with the challenging atypical case, and effective patient counseling. It teaches principles, including the role prostaglandins play in both conditions, as well as procedures, like surgical options and TENS, providing a balance of the practical and the esoteric. The inclusion of patient education and counseling materials additionally increases its usefulness for the busy clinician. This is an ideal guide for ob/gyn clinicians, residents, and trainees caring for patients with these common, but often misunderstood diseases.

Recurrent Implantation Failure: Etiologies and Clinical Management

by Jason M. Franasiak Richard T. Scott Jr.

This unique text discusses the wide range of causes and pathophysiologic conditions contributing to recurrent implantation failure (RIF) and the current treatment approaches to best approach this challenging patient population. Beginning with a presentation of the mechanism of normal implantation, the physiologic synchrony between embryo and endometrium, and the roles of both gametes and embryos in RIF, the book then examines the various genetic, immune and environmental factors involved. Abnormalities of the hematologic and endocrine system, as well as those of the anatomy and microbiome, are described in detail. Last but no less important, the psychological stress and treatment of patients experiencing RIF is discussed. Assisted reproductive technology (ART) success has increased substantially over the past three decades. However, despite these advances, recurrent implantation failure is still a common issue confronting patients and clinicians, and RIF represents one of the greatest challenges in ART and remains a source of frustration and disappointment for patients, clinicians, and researchers involved in the process. Providing an in-depth investigation into the pathophysiology and biological mechanisms of RIF and its clinical management, this will be an excellent resource for reproductive endocrinologists and infertility specialists.

Newton’s Sensorium: Anatomy of a Concept (Archimedes #53)

by Jamie C. Kassler

These chapters analyze texts from Isaac Newton’s work to shed new light on scientific understanding at his time. Newton used the concept of “sensorium” in writings intended for a public audience, in relation to both humans and God, but even today there is no consensus about the meaning of his term. The literal definition of the Latin term 'sensorium', or its English equivalent 'sensory', is 'thing that feels’ but this is a theoretical construct. The book takes readers on a process of discovery, through inquiry into both Newton’s concept and its underlying model. It begins with the human sensorium. This part of his concept is situated in the context of the aforesaid writings but also in the context of the writings of two of Newton's contemporaries, the physicians William Briggs and Thomas Willis, both of whom were at the forefront of their respective specialties of ophthalmology and neurology. Only once the human sensorium has been explored is it possible to generalize to the unobservable divine sensorium, because Newton's method of reasoning from experience requires that the second part of his concept is last in the order of knowledge. And the reason for this sequence is that his method, the short-hand term for which is 'analogy of nature', proceeds from that which has been observed to be universally true to that which is beyond the limits of observation. Consequently, generalization passes insensibly into reasoning by analogy.Readers will see how certain widespread assumptions can be called into question, such as that Newton was a theological voluntarist for whom the will is superior to the intellect, or that, for Newton, not only the world or universe but also God occupies the whole extent of infinite space. The insights afforded through this book will appeal to scholars of the philosophy of science, human physiology, philosophy of mind and epistemology, among others.
