The aim of this book is to bridge the gap between standard textbook models and a range of models where the dynamic structure of the data manifests itself fully. The common denominator of such models is stochastic processes. The authors show how counting processes, martingales, and stochastic integrals fit very nicely with censored data. Beginning with standard analyses such as Kaplan-Meier plots and Cox regression, the presentation progresses to the additive hazard model and recurrent event data. Stochastic processes are also used as natural models for individual frailty; they allow sensible interpretations of a number of surprising artifacts seen in population data. The stochastic process framework is naturally connected to causality. The authors show how dynamic path analyses can incorporate many modern causality ideas in a framework that takes the time aspect seriously. To make the material accessible to the reader, a large number of practical examples, mainly from medicine, are developed in detail. The European School of Oncology came into existence to respond to a need for information, education and training in the field of the diagnosis and treatment of cancer. There are two main reasons why such an initiative was considered necessary. Firstly, the teaching of oncology requires a rigorously multidisciplinary approach which is difficult for the Universities to put into practice since their system is mainly disciplinary orientated. Secondly, the rate of technological development that impinges on the diagnosis and treatment of cancer has been so rapid that it is not an easy task for medical faculties to adapt their curricula flexibly. With its residential courses for organ pathologies and the seminars on new techniques (laser, monoclonal antibodies, imaging techniques etc.) or on the principal therapeutic controversies (conservative or mutilating surgery, primary or adjuvant chemotherapy, radiotherapy alone or integrated), it is the ambition of the European School of Oncology to fill a cultural and scientific gap and, thereby, create a bridge between the University and Industry and between these two and daily medical practice. One of the more recent initiatives of ESO has been the institution of permanent study groups, also called task forces, where a limited number of leading experts are invited to meet once a year with the aim of defining the state of the art and possibly reaching a consensus on future developments in specific fields of oncology.

Reeds van oudsher speelt de tandarts een belangrijke rol bij de preventie en de bestrijding van de negatieve gevolgen van chemische en bacteriologische bedreigingen van het kauwstelsel. De infecties en ontstekingen die hier het gevolg van kunnen zijn, tasten de gezondheid van de tanden en kiezen (cariës, endodontitis) en de omringende weefsels (parodontitis) aan. Echter, steeds duidelijker wordt dat ook mechanische bedreigingen de gezondheid van het kauwstelsel ernstig kunnen schaden. Verkeerde mondgewoonten zoals tandenknarsen en klemmen (bruxisme) kunnen een ongestoorde en pijnvrije functie van de kauwspieren en kaakgewrichten belemmeren (temporomandibulaire disfunctie en orofaciale pijn), aanleiding geven tot ernstig verlies van tandweefsel (gebitsattritie) en een bedreiging vormen voor tandheelkundige restauraties in de mond. Een verkeerde stand van de onderkaak en de tong tijdens de slaap kan aanleiding geven tot het obstructieve slaapapneusyndroom (OSAS).
