This book owes its existence to an ideal, a burning frustration, and a trusted believer. The ideal was the sense that governed my feelings about systematic desensitization during my early introduction to its benefits. It is hard to put into words the initial doubts that pervaded me during my first attempt with desensitization with a seriously phobic client, as I re­ ligiously worked my way through the procedure: "Will this client really become relaxed? And then what-will the visualization actually occur? And then what-will the fear really vanish, just like that?" And oh, the feeling of discovery, and validation, when indeed the process worked, and worked well. Desensitization was everything it was claimed to be: systematic, clean, theoretically grounded, empirically tested, applicable as a behavioral technology regardless of one's own theoretical bias. And there were testable outcomes; concrete evidence for change. So I became invested and aimed at doing more with desensitization. My students and I raised some theoretical questions in order to open the doors for revising the desensitization to improve on its applications. We tested the rapidity with which desensitization could be accomplished, shortening the time by shortening the anxiety hierarchy. Along with others, we studied the question of group delivery, and reducing the total number of sessions, as well as examining the use of audiotaped delivery of services.

For over thirty years the benzodiazepines monopolised not only the anxiolytic market but also clinical and animal research in anxiety. Indeed many animal tests developed since the 1960s have been optimised for the benzodiazepines and some programmes have even screened candidates as potential anxiolytics on their benzodiazepine-like side-effects rather than their anxiolytic activity. With the realisation of the drawbacks of the benzodiazepines, namely their potential for tolerance and dependency, there has been a renewed interest in alternative anxiolytics both from existing drugs such as the tricyclic and monoamine oxidase antidepressants and from newer agents such as buspirone. In addition anxiety is no longer considered to be a unique entity but rather an umbrella term for a series of specific anxiety disorders such as panic disorder without or with agoraphobia, generalised anxiety disorder (GAD), specific phobias, social phobias and post-traumatic stress disorder (PTSD). These new clinical categories have opened another dimension in the therapy of anxiety requiring the optimisation of treatments for different syndromes. This book is a critical review of today's anxiolytics and those that may become the anxiolytics of tomorrow. What is clear is that currently there are few clinically satisfactory alternatives to the benzodiazepines for the treatment of acute anxiety. For chronic anxiety, it is generally agreed that benzodi­ azepines are not the treatment of first choice. The tricyclic and monoamine oxidase antidepressants, the serotonin reuptake inhibitors and buspirone offer better solutions for chronic anxiety but they are still far from being ideal.

AO/ASIF Instrumentation: Manual of Use and Care

During their 20 years of activity members of the Associa­ tion for the Study of Internal Fixation (AO - ASIF) have made authoritative contributions to the development of internal and external fixation. The close collaboration of surgeons, basic researchers, metallurgists, engineers and the establishment of clinical documentation has made it possible to achieve a solid scientific basis for internal fixa­ tion. Clear definitions for the standardization of different types of osteosynthesis were possible: interfragmentary compression, splintage and buttressing as well as combina­ tions of these three techniques. At the same time a scienti­ fic and workmanlike instrumentation was developed. The idea was to keep diversification within limits but, however, to assemble a comprehensive collection of implants and in­ struments to answer all the problems presented by the com plexity of bone operations. Osteosynthesis is a difficult and demanding operative method. Its. claims on the surgeon and the theatre staff are high. Therefore plans have existed for a long time to supplement the "Manual of Internal Fixation" with a de­ tailed description of the AO Instrumentation, its use and maintenance. Our collaborator FRIDOLIN SEQUIN, graduate engineer, has accomplished this task with expert knowledge. He has organized over many years courses for theatre nurses and has been able from the resulting experience to provide helpful suggestions. When RIGMOR TEXHAMMAR R. N. joined AO-International four years ago, it was natural to include her as a co-author.

AO/ASIF Instruments and Implants: A Technical Manual

The original AO/ASIF Instrumentation manual presented a concise and complete description of the AO instruments. Thoughtfully developed by Fridolin Sequin and Rigmor Tex­ hammar, the manual discussed in a clear fashion the purpose and care of the various AO instruments that are handled by the operating room staff. One important feature of the first edition was a detailed checklist of the instruments required for the more common operative procedures for treating fractures. Fridolin Sequin was well-suited to author the first edition: his 15 years of experience as a technical engineer for the AO gave him in-depth knowledge of AO instruments, and he drew on the clinical knowledge of Rigmor Texhammar, a consultant and di­ rector of the AO courses for nurses. Its original feature of com­ bining a column of text with a column of illustrations meant the manual quickly became accepted as a standard. By 1981, trans­ lations could be found in English, French, Spanish, and Italian. Not surprisingly, the manual was very popular.

Aorta und große Arterien: Pathophysiologie, Klinik, Röntgenologie und Chirurgie

Viele grundlegende experimentelle Erkenntnisse der Gefäßchirurgie, der Ausarbeitung der Nahttechnik und der Gefäßtransplantation reichen in die Jahrhundertwende zurück. Zur Überbrückung von Defekten an großen peripheren Arterien haben sich die auto­ plastischen Venentransplantate seit ihrer ersten klinischen Anwendung durch LEXER 1907 bisher am besten bewährt. Durch zahlreiche arteriographische Untersuchungen wurde an der Rehnschen Klinik in Freiburg i. Br. anhand großer Erfahrungen aus dem zweiten Weltkrieg der Beweis für die Durchgängigkeit der verpflanzten Venen erbracht. Obwohl die ersten Versuche der Arteriohomoioplastik schon kurz nach der Jahrhundertwende ausgeführt wurden, blieb das Verfahren ohne klinische Bedeutung, bis R. E. GRoss u. Mitarb. 1948 die ersten erfolgreichen Transplantationen bei der Aortenisthmusstenose und bei der Fallotschen Tetralogie mitteilten. Damit waren auch Transplantationen an der Aorta und den großen Stammgefäßen möglich. Durch die Weiterentwicklung der klinischen und röntgenologischen Diagnostik, die bekannten Fortschritte der allgemeinen Chirurgie und Anaesthesie und durch die erweiterte klinische Anwendung der Arteriohomoioplastik erlebte die Chirurgie der Aorta und der großen Arterien einen neuen Aufschwung und gewann zunehmende Bedeutung in Klinik und Forschung.
