As indicated in the Preface, the contributions to this volume are based upon the papers presented at the symposium on Thermoreceptors and Temperature Regula­ tion held in July 1988 at the Institute of Physiology of the University of Marburg (Federal Republic of Germany) to celebrate and commemorate the life and achievements of HERBERT HENSEL, who directed that Institute from 1955 until his death in 1983, and whose most notable and significant contributions to thermo­ physiology were in the areas of the properties and characteristics of thermo sensors, mammalian thermoregulation more generally, and the psychophysiology of ther­ mal sensation. All the papers in this volume deal, to a greater or lesser extent, with these discernibly different but closely allied aspects of mammalian physiology. The editors have sought to achieve cohesion, flow, and balance both in the contributed articles and in their order of presentation, without either large gaps or redundancies in the coverage of the recent advances in the understanding of thermoreceptors and thermoregulation. At the same time we have sought to avoid such a degree of editorial control as to destroy the individuality of the contributions, and the judgements upon which they were based. We have also sought to look both backwards and forwards, and to include some legitimate extension of the con­ sideration of thermosensitivity and thermoregulation into such areas as climatic adaptation and fever. Hence the "greater or lesser" of the closeness of this series of papers to HERBERT HENSEL'S scientific interests.

Exercise testing is widely used all over the world to assess functional capacity in athletes, healthy subjects and patients. According to recent surveys, the interest in ergometry is still growing in almost all fields of medicine, especially in private practice. Furthermore, there has been an exponential growth in the number of publications on exercise testing in the last years. Several consensus and task force conferences have dealt with exercise testing and published recommendations on standardization and guidelines in ergometry. These factors have, in combination, initiated an upsurge in research and clinical use of exercise testing. At the 6th International Seminar on Ergometry the latest findings and advances in ergometry were discussed. Reviews and results of the congress covering a wide range of features in exercise testing are presented in this book. Letztere unterscheidet sich sehr wesentlich von den bisher beschriebenen Techniken, da nicht eine rotierende Fräse, sondern eine oszillierende Feile zur Knochenresektion verwendet wird. Das Buch vermittelt, beginnend bei der Indikationsstellung zur Arthroskopie bzw. arthroskopischen Therapie, über Lagerung, Abdeckung, notwendige Instrumente bis hin zur technischen Durchführung der Arthroskopie selbst, alle notwendigen Einzelheiten, die für die Schulterarthroskopie erforderlich sind.

Computerized Cardiopulmonary Exercise Testing

The measurement of cardio-circulatory and gas-exchange parameters during phy­ sical exercise - the so-called ergo spirometry or cardiopulmonary exercise testing (CPX) - as a basis of pathophysiological and clinical research has a long tradition in Cologne. Knipping and his coworkers, especially Hollmann, performed basic re­ search work in healthy subjects. In the area of sports medicine, bicycle or treadmill exercise testing with parallel serial lactate determinations has gained increasing im­ portance for the assessment of cardiac functional capacity. Also, in other medical disciplines, ergospirometry lost its importance. K. Wasserman in Los Angeles is to be credited for having further improved the method to its present standard, a computerized, on-line measuring and practicable cardiopulmonary exercise testing procedure. The prerequisites were technical innovations, such as continuously­ measuring gas analyzers and personal computers. Thereby, the knowledge about physiology, pathophysiology, and clinical circumstances of cardiocirculatory and re­ spiratory regulation during exercise were significantly extended. The working groups of W. Hollmann, Cologne, and K. Wasserman, Los Angeles, determined normal values for the gas-exchange parameters and derived values for healthy normals in large populations. Wasserman and coworkers were able to introduce a differential diagnostic concept for patients suffering from various cardiovascular and cardio­ pulmonary diseases. Many cardiologists, working, for example in myocardial failure or with rate-adaptive pacemakers, belong to those who recommended the modem, computerized ergo spirometry. Furthermore, this method is controversely discussed· by colleagues working in sports medicine and pulmonary function.

Geahnt und befürchtet hatten die Sportbegeisterten und -zuschauer schon seit langem, daß im Leistungssport gedopt wird. Trotz gelegentlicher Selbstbekenntnisse von Sportlern aber wurde dies von offizieller Seite stets bestritten und Vermutungen als bösartig und falsch abgetan. Noch bestand Hoffnung, daß Fairness und natürliche Freude am Wettkampf im Spitzensport ausschlaggebend sei. Erst als sich deutliche Hinweise - u.a. Ben Johnson in Seoul - nicht mehr abstreiten ließen, fanden in den USA, in Canada und Australien offizielle Untersuchungen statt. Bei den dabei unter Eid gemachten Aussagen von Sportlern, Trainern, Sportmedizinern und Funktionären kam das ungeheure Ausmaß des Dopingmißbrauchs langsam ans Tageslicht. Auch in der Bundesrepublik wurde eine Kommission eingesetzt, deren nun vorliegender Bericht zeigt, daß zumindest in der ehemaligen DDR flächendeckend gedopt wurde. Die offizielle Empfehlung lautet allerdings: keinen Schuldigen nennen, schweigen - und schweigend weiterdopen, um auch bei den nächsten internationalen Sportfesten glänzen zu können. Ohne die genauen Fakten und Namen der Verantwortlichen und Mittäter zu nennen, wird es aber keine Reinigung des Sports vom Doping und keine Zukunft für einen menschenwürdigen Leistungssport geben. Noch gelten die Anabolika-unterstützten Rekorde als Vorgaben. Deshalb und besonders auch der zahlreichen Opfer wegen - der ohne ihr Wissen mit Hormonen behandelten oft jugendlichen Sportler auf der einen Seite sowie derjenigen, die vergeblich versucht hatten, auf faire Art sportliche Leistungen zu erbringen - hat sich die Autorin entschlossen, mit diesem Buch die Wahrheit an die Öffentlichkeit zu bringen. Dokumentiert wird auch, welche gesundheitlichen und psychischen Schäden durch Dopingmittel (androgene Hormone) besonders im Frauensport angerichtet werden, und wie Sportmediziner und Wissenschaftler mitgewirkt haben an Menschen-Versuchen und Forschungsvorhaben mit dem Ziel, neue, stärkere Mittel zu entwickeln und Dopingkontrollen effektiver zu umgehen. Brigitte Berendonk war viele Jahre erfolgreich im Spitzensport - u.a. Deutsche Jugendmeisterin in der damaligen DDR, dann in der BRD, später Deutsche Meisterin im Diskuswerfen und Kugelstoßen und zweifache Olympiateilnehmerin. Da dessen Sekretion in der Mehrzahl der Fälle die Depoteisenreserven korrekt wiederspiegelt, ermöglicht dies eine schnelle und sichere Diagnose, insbesondere von Eisenmangelzuständen. Wegen der Häufigkeit des Eisenmangels und der meist einfachen Therapie sollte diese Erkenntnis zum medizinischen Allgemeingut gehören. Das vorliegende Buch trägt zu einem besseren Verständnis der wesentlichen pathophysiologischen Grundlagen und diagnostischen Prinzipien bei.

Disturbance of iron metabolism, particularly iron deficiency, are among the most commonly overlooked or misinterpreted diseases. This is due to the fact that the determination of transport iron in serum or plasma, which is the test used in conventional diagnosis, does not allow a representative estimate of the body's total iron reserves. A proper estimate was formerly possible only by the costly and invasive determination of storage iron in the bone marrow; sensitive, well-standardized immunochemical methods for the precise determination of the iron storage protein ferritin in plasma are now available. Since the secretrion of this protein correctly reflects the reserves of depot iron in the majority of cases, these methods permit fast and reliable diagnoses, particularly of iron deficiency conditions. The fact that iron deficiency is so common and is usually simple to treat ought to be well known in the medical world. This book is intended to contribute to a better understanding of the main pathophysiological foundations and diagnostic principles.
