A Medical Educator's Guide to Thinking Critically about Randomised Controlled Trials: Deconstructing the "Gold Standard"

Drawing on the statistical and philosophical expertise of its authors, this book is designed to improve understanding and use of randomised controlled trials (RCTs) among health professionals. It is intended for use primarily by medical educators involved in teaching statistics and evidence-based medicine (EBM) to medical students, junior doctors and other health professionals. However, each of the chapters serves a wider range of interests, including the practical needs of physicians in interpreting research evidence to support clinical decision making and the teaching needs of philosophers of medicine who want to more fully appreciate how RCTs work in practice and provide engaging examples for their students. Rather than compete with the proliferating methodological literature on RCT designs, this book focuses on cultivating a healthy skepticism among developing health professionals to support critical appraisal of their own and published work on RCTs at a fundamental level, including through a more informed understanding of the place of subgroup analyses in sound statistical inference. Management of the positive predictive value in the statistical analysis of RCT findings is included as an important topic for contemporary medical curricula. In comparing RCTs with non-randomised studies, a search for empirical evidence for the superiority of RCTs is initiated, pointing to the need for further work to confirm what form this evidence should take.Medical educators will find a wealth of reasons to encourage their students to think more critically about how the RCT operates in practice as a gold standard.

Menacing Tides: Security, Piracy and Empire in the Nineteenth-Century Mediterranean

New ideas of security spelled the end of piracy on the Mediterranean Sea during the nineteenth century. As European states ended their military conflicts and privateering wars against one another, they turned their attention to the 'Barbary pirates' of Algiers, Tunis and Tripoli. Naval commanders, diplomats, merchant lobbies and activists cooperated for the first time against this shared threat. Together, they installed a new order of security at sea. Drawing on European and Ottoman archival records – from diplomatic correspondence and naval journals to songs, poems and pamphlets – Erik de Lange explores how security was used in the nineteenth century to legitimise the repression of piracy. This repression brought European imperial expansionism and colonial rule to North Africa. By highlighting the crucial role of security within international relations, Menacing Tides demonstrates how European cooperation against shared threats remade the Mediterranean and unleashed a new form of collaborative imperialism.

In the first half of this book, the author tackles debates about the content of perception, namely, what sorts of properties or features of the world reveal themselves to us in perception and in what modes. Drawing on Merleau-Ponty’s description of perceptual “sense,” the author argues that perception has a unique kind of content, which cannot be adequately described in terms of sensations or concepts. He then shows how this account of perceptual sense can clarify debates about the richness of perceptual content, including whether we can perceive moral properties. In the second half, he turns to the nature of perception. Here he argues that Merleau-Ponty’s account of perceptual intentionality makes available a powerful combination of the core insights of two main contemporary approaches to this question: realism and intentionalism. Students’ difficulties in producing texts that meet the requirements of academic writing are a recurring concern for teaching staff and those responsible for university courses. Various initiatives are currently being taken, mainly at undergraduate level, to help students improve the quality of their writing. Research into metacognitive processes and the self-regulation of learning can be used to support the design of these writing support systems, particularly by providing a better understanding of the students’ difficulties. This book reviews the concepts of metacognition and self-regulation in relation to writing processes. It analyses the metacognitive components involved in text production, their links with successful writing and their individual and contextual determinants. It completes this analysis by drawing on the teaching and assessment of writing in higher education. All of these elements are articulated around a multifactorial modeling of the learning and teaching of academic writing.

The Metaphysics of Meditation: Sri Aurobindo and Adi-Sakara on the Isa Upanisad (Bloomsbury Studies in World Philosophies)

In this book Stephen Phillips focuses on one of the most important poems about meditation in world literature, as understood by two of the greatest philosophers of India, one classical, one modern. Sankara's commentaries on the Upanisads are a core of the Vedanta tradition and Aurobindo is a towering figure of 20th-century Hindu thought. This is the first time their approaches have been studied together.The Isa (c. 500 BCE) an “Upanisad” belongs to a genre of “adhyatmika” learning-concerning self and consciousness-in early Indian literature. According to the Ancient Indian tradition of yoga, meditation is antithetical to willful bodily and mental action. Breathing is all you do. In the conception of the Isa Upanisad, we are told that the best that comes from meditation is because of what the “Lord” is. In Sankara's interpretation it comes to block out the little “you,” whereas according to Aurobindo it comes as a divine connection, an occult “Conscious Force” belonging to truer part of oneself, atman, and an “opening” to that self's native energy. Framed around Aurobindo's translation of each of the Isa's eighteen verses, along with a translation of each verse, Phillips follows a different reading of Sankara as laid out in his commentary. All this is done against the backdrop of modern scholarship. Convergences and divergences of these streams are the focus throughout. Appendix A presents the Upanisad with the two readings side by side.This book traces a worldview and consonant yoga teaching common to two authors who are typically taken to be oceans apart, not only chronologically but in intellectual stance. Addressing a huge gap in the contemporary literature on meditation in the Hindu traditions, Phillips presents a compelling new way of thinking about meditation in the Advaita Vedanta philosophy and Upanisad.

The Metaphysics of Michael Polanyi: Toward a Post-Critical Platonism

This book tells the story of how the Platonic vision of Michael Polanyi – the Hungarian-British chemist and philosopher – bridges the gap between speculative metaphysics and scientific practice, thus making sense of the broad swathe of human experience in a phenomenologically satisfying fashion. The central proposal is that Polanyi is a Platonist due to his affirmation of the ontological status of abstract objects, with particular focus placed on the question of uninstantiated universals. This is an open access book which focuses on different aspects of education, employment, and successful integration of migrants in three countries: Norway, Sweden, and Switzerland. The chapters in this book reflect on these issues from micro, meso and macro perspectives; some are based on interviews with migrants and people who work with them, others on documents and literature about migration. There are different pathways for skilled migrants to vocations. Some start working in their previous vocations after arriving in the new environment. Some re-enter their professions but on a lower level. Some can re-train themselves in a new vocation, and some will go to further education, as studies in different chapters of this book suggest. Common for successful integration seems to be several intertwined factors: the target language competence, strong motivation and agency, supporting networks and supporting persons, as well as structural opportunities of the new environment. The book’s editorial board takes an eclectic view, hoping to start an academic debate about what ‘successful integration’ means. While discussions about the integration of migrants tend to focus on integration failures, there are millions of migrants, in different countries, who have successfully integrated into their new societies.

In 2008 an Iraqi artist was waterboarded as performance art. In 2010 artists upturned police cars in Russia. But what exactly do we mean by militant art and aesthetics? Combining these examples with the pioneering thought of Badiou, Žižek, Rancière and Mouffe, as well as up-to-date scholarship from Bishop, Léger and others, Lang investigates the instances, attributes and rules of militant art in order to introduce a new overall theory of 21st-century militant aesthetics.

Military Necessity and Just War Statecraft: The Principle of National Security Stewardship (War, Conflict and Ethics)


This book analyses the concept of military necessity and just war thinking, and argues that it should be seen as a vital moral principle for leaders. The principle of military necessity is well-understood in the manuals of modern militaries and is recognized in the war convention. It is the idea that battlefield commanders should make every effort to win on a local battlefield, within legal means, and using proportionate and discriminating weapons and tactics. Every legal textbook on war includes military necessity as a foundational principle within the jus in bello (ethics of fighting war) alongside principles of proportionality and distinction, and it is taught in every Western military academy. Even the International Committee of the Red Cross lauds the concept as a cardinal principle of warfare. However, unlike legal scholarship, pick up a book by almost any just war thinker in philosophy, theology, or the social sciences, and the concept is missing altogether. This volume returns military necessity to just war thinking and lays out the argument for doing so. Each contributor taps into one of the many dimensions of military necessity, such as its relationship to jus ad bellum (ethics of going to war) categories (e.g. right intention), its relationship to jus in bello categories, or its application in foreign policy and military doctrine. Case studies in the book point out the practical moral dimensions of military necessity in cases from the targeted killing of terrorists to battlefield decisions that led to the use of the atomic bomb at Hiroshima. This book analyses the concept of military necessity and just war thinking, and argues that it should be seen as a vital moral principle for leaders. The principle of military necessity is well-understood in the manuals of modern militaries and is recognized in the war convention. It is the idea that battlefield commanders should make every effort to win on a local battlefield, within legal means, and using proportionate and discriminating weapons and tactics. Every legal textbook on war includes military necessity as a foundational principle within the jus in bello (ethics of fighting war) alongside principles of proportionality and distinction, and it is taught in every Western military academy. Even the International Committee of the Red Cross lauds the concept as a cardinal principle of warfare. However, unlike legal scholarship, pick up a book by almost any just war thinker in philosophy, theology, or the social sciences, and the concept is missing altogether. This volume returns military necessity to just war thinking and lays out the argument for doing so. Each contributor taps into one of the many dimensions of military necessity, such as its relationship to jus ad bellum (ethics of going to war) categories (e.g. right intention), its relationship to jus in bello categories, or its application in foreign policy and military doctrine. Case studies in the book point out the practical moral dimensions of military necessity in cases from the targeted killing of terrorists to battlefield decisions that led to the use of the atomic bomb at Hiroshima.
